The Community Needs Assessment Survey is being collected and analyzed by SA Howell, LLC (an independent consulting firm) on behalf of CSP, your local community action agency.

 

The results of this survey will be used, along with other research, to help CSP identify the primary causes and conditions of poverty so that agency strategies can be designed to promote change.

Your name and contact information is requested to prevent duplications and to allow our consulting firm to follow up with questions (if necessary).  At no time will your name be provided, in association with your answers, to CSP.
